APPLICATION DECLARATION Our Faith Commitment
We as parents/guardians hereby request that our child be enrolled to receive the sacraments of Confession and Communion in Sandyford Parish. We promise to do our best to create a Christian environment in our home and to renew our own commitment to the faith of the Catholic Church, in particular through attendance at Mass. We accept, as we did when our child was baptised catholic, that the primary responsibility in handing on the faith is ours as parents/guardians. We want to accept the help of our local church and school in living up to this responsibility.
You MUST know the name of the Parish where your child was baptised and the DATE of the BAPTISM in order to complete this enrolment form. Candidates MUST be baptised according to the rites of the Catholic Church.

The Parish would be grateful for this or any contribution you feel you are in a position to make either now or whenever you may be in a position to do so. *Please email sandyfordholycommunions@gmail.com, before submitting the second Form, if you have more than one child registering for First Holy Communion and/or Confirmation at this time.

Acknowledgement of Receipt of Submitted Form
I understand that this enrolment will not be complete until I receive an automated response email from us and that if I do not receive this acknowledgement immediately I understand that it is my responsibility to inform the Sacramental Programmes Coordinator at sandyfordholycommunions@gmail.com

Dates for Sacraments 2027
The specific dates for First Holy Communion Masses and Confirmation Masses for each school are notified to all enrolled families as soon as possible once all dates have been finalised and agreed (between Archbishop’s House Dublin Diocese, all Parish Schools and the Parish). This usually occurs in the October prior to the year of the Sacraments, i.e. October 2026 for Sacraments 2027.

The information contained in this Form may be used to communicate with parents/guardians in preparation for the Sacrament, and to register the Sacramental details in the Parish Records. The information will be accessed by the Parish Sacramental Team only. The information submitted in this form will be destroyed within a year of the Sacrament being received and recorded in the Parish Registers. The information in the Parish Register will be retained permanently.
